- He made his first film appearance by playing the role of a driver in the 2003 Telugu-language drama film Sambaram, starring Nithiin and Nikitha in lead roles. However, Nikhil’s role went uncredited.
- Alongside Varun Sandesh, Rahul Haridas, Vamsee Krishna, Tamannaah, Sonia Deepti, Gayatri Rao, and Monali Chowdhary, Nikhil Siddhartha co-starred as Rajesh in the 2007 Telugu-language musical coming-of-age film Happy Days. A low-budget film, the film went on to become one of the most successful movies of 2007.
- He was cast as Karthikeya “Karthik” Kumaraswamy (a medical student who does not believe in supernatural powers and analyzes everything using science and logic) in the 2014 Telugu-language mystery thriller film Karthikeya.
